Why Do I Need to be Prequalified?
Shiel Sexton requires all potential and current Trade Partners to complete our prequalification annually. An approved prequalification is required prior to providing services on Shiel Sexton projects. The process is secure and confidential.
What Documents Will I Need?
- OSHA No. 300A Logs
From the Past 3 Years - Experience Modification Rate Document (EMR)
From the Past 2 Years + Current Year - Program/Policy Documents (If Available):
- Safety Program
- Drug & Alcohol Testing Policy
- New Employee Orientation Plan
- Return to Work Program
- Hazard Communication Plan
- OSHA No. 300A Logs
- Audited/Reviewed/Compiled Financial Statements
For the Past 2 Years - Latest Balance Sheet, Income Statement, & Footnotes
- Form W-9
Showing full legal name, signed & dated within the past year
Note Regarding Financial Statements:
An NDA can be signed if needed prior to the submittal of financial statements. Please email your NDA to prequalification@shielsexton.com, or use the NDA form we have here.
